Our highly trained and qualified Registered and Licensed Practical Nurses deliver care to your residence, home or Assisted Living. We provide care after an illness, injury, recent hospitalization or a visit to your physician’s office.  In most cases, services are reimbursed by Medicare, Medicaid, and Private Insurance.

Our nursing services include:

• In-home nursing assessments and evaluations as needed

• Medication review, education, and organization

• Care of surgical wounds, dressings, feeding tubes, and care and  instruction with foley catheters and drains

• Instruction and monitoring of disease process

• Nutritional Counseling

• Telehealth- remote monitoring

Pediatric Care

Meridian At Home provides nursing and rehabilitative therapy right where children want to be, surrounded by their family and favorite things. We deliver family-centered home care to children with special health needs in the comfort of their home. Care plans are coordinated by a registered nurse in collaboration with each child’s physician, and team meetings are held as needed with parents and caregivers to answer questions and refine treatment goals.

Maternal-Child Health Services

Devoted to mothers and infants, our preinatal nurses provide home services such as infertility therapy, prenatal education, pre-term birth prevention, monitoring for high-risk pregnancies, breast feeding, and infant care instruction. Our nurses work in close collaboration with obstetricians, perinatologists, and pediatricians to help with all issues and concerns.

Infusion Therapy

Our Infusion therapy program gives patients the unique opportunity to receive intravenous medications and fluids while relaxing at home making it possible to shorten or eliminate hospital stays.  Learn more about our Infusion services

Wound/Ostomy Care

Our certified enterostomal and wound care nurses provide wound care evaluation and surgical debridement, assess and instruct in all stoma and wound care, and assist in ordering supplies.

Palliative Care

Our team of trained professionals will offer comforting care and focus on effective symptom management in collaboration with your physician. The care is individualized but may include pain management and symptom relief and also provide training for you- the caregiver on ways to manage your loved ones pain and symptoms.

All of our specialty nurses will collaborate with your physician and other members of our team to provide a wide range of therapeutic services.

In addition to nursing care, Meridian At Home offers Certified Home Health Aides that are available to assist the nurses with delivery of care.

Physical Therapy

Meridian At Home’s licensed physical therapists promote healing, improved movement and function. Occupational Therapy

Our licensed occupational therapists treat a variety of developmental, physical, emotional, and social issues. They enable our patients to achieve their highest level of function in their home to perform their activities of daily living.

Speech and Language Pathology

Specially trained in speech and swallowing disorders, our speech pathologists assist patients in regaining the ability to produce and understand speech, as well as swallowing assessment, training and skill acquisition.  They treat a variety of impairments associated with neurological conditions (including head injury and strokes), hearing loss, head and neck surgery, and developmental disorders.

Home Orthopedic Program-Gold Standard

Skip the traditional rehabilitation center after your orthopedic surgery, like knee or hip replacement, and start your rehabilitation process right in your own home!  With our Gold Standard orthopedic rehabilitation program, patients receive one-on-one therapy that begins within 24 hours after hospital discharge. This unique program fosters a partnership between the rehabilitation team and the orthopedic surgeons. Together with the skilled delivery of post-surgical care, this translates into a decreased risk of hospital readmission and better functional results and a faster recovery for the patient.
