If you or your loved one have health concerns but are fully able to care for yourself, the Clinical Connections Solution can provide you with safety, security and peace of mind. Using cutting edge technology, your health concerns are monitored remotely. If there is a change in your condition, a registered nurse calls you directly. It's that simple. It's that easy.
What's included:
- An in-home consultative assessment by a registered nurse
- An RN will come to you or your loved one's home to discuss ways to prevent accidents along with methods for monitoring activity to assure peace of mind and safety. They will also work with you, your loved one and their physician to create a plan that details the services to be provided. This plan will include financial arrangements, insurance coverage information and the course of treatment including the caregiver's responsibilities.
